You agree to provide complete, accurate, and truthful information during consultations.

You understand that healthcare providers may:

  • Request additional documentation
  • Require additional evaluations
  • Refer you to another provider
  • Recommend in-person treatment
  • Decline certification

based on available information and professional judgment.

7. NO GUARANTEE OF CERTIFICATION OR APPROVAL

If the healthcare provider is unavailable, unable to accommodate the appointment, or the appointment cannot be completed due to provider-related issues, the patient may be eligible for a full refund in accordance with applicable policies and law.

A missed appointment or failure to attend a scheduled consultation does not automatically result in forfeiture of services, and the Company may, at its discretion, permit the patient to reschedule or complete the appointment at a later time.

Refunds may be denied when:

  • Services have already been rendered.
  • A healthcare provider has completed an evaluation.

Additional forms, letters, certifications, employment documents, legal documents, disability paperwork, housing forms, accommodation requests, or other supplemental documentation requested by a patient are not included in the cost of a certification appointment.

Healthcare providers retain sole discretion to determine whether they are willing to complete any requested form or documentation. Providers may decline any request for additional documentation for any lawful reason, including professional judgment, legal concerns, insufficient information, or scope-of-practice considerations.

Fees for additional forms or documentation are determined solely by the healthcare provider and may vary depending on the complexity, time requirements, and nature of the request. No fee quote, estimate, or guarantee shall be provided until the healthcare provider has reviewed and agreed to complete the requested documentation.

Payment for additional documentation must be received before any form, letter, certification, or supplemental documentation is completed or released.

While many approved documentation requests are completed within forty-eight (48) to seventy-two (72) hours, completion times are estimates only and are not guaranteed. Certain requests may require additional review, supporting documentation, provider availability, or other considerations and may take longer to complete.
