Ohio Patients can receive a 50% discount on the annual $50 certification fee if they qualify under veteran or disability status.
To qualify under veteran status, provide a copy of one of the following :
1. Department of Defense Identification Card
2. DD214, DD215, or National Guard Bureau military discharge certificate indicating disposition of discharge
3. Report of separation from the National Archives National Personnel Records Center in St. Louis, Mo
4. Veterans Identification Card from the Department of Veterans Affairs.
All acceptable proof documents, except a Veterans Identification Card, must show the veteran status as honorable, general, general under honorable conditions, or discharged or released under conditions other than dishonorable. Documentation must also include the applicant’s date of birth, and may be emailed to MMCPRegistry@pharmacy.ohio.gov or mailed to: State of Ohio Board of Pharmacy MMCP Patient Registry 77. S. The amount of monthly SSDI or SSI benefits the patient receives during the year or a state retirement program showing monthly benefits
